The Lakehead NPLC is hosting a Memory Screening Clinic

JUNE 12, 2013

Please call 475-9595 to book an appointment with us!

We hope to see:

Anyone age 75 or over

Anyone ages 65-74 with:

Diabetes
Heart disease
A previous heart attack or stroke
High blood pressure
A history of smoking
High cholesterol
Atrial fibrillation
Obesity
A family history of heart attack or stroke before the age of 60

Participants will be screened by a team which includes a Nurse Practitioner or Registered Nurse, our Social Worker and our Pharmacist.

Follow-up appointments may also be booked for you with our dietitian.

Please bring all of your medications and a family member to your appointment!

Physiotherapist/Kinesiologist: Lakehead NPLC is seeking a part-time physiotherapist or kinesiologist to work in collaboration with primary care providers to address patient concerns in mobility and injury recovery with an emphasis on frailty and chronic disease management. This is a contract position until March 31, 2025, with an option to renew.
