Best Care
The Best Care Program provides complete respiratory assessments and education by a Respiratory Therapist in clinic for patients with respiratory conditions such as asthma and COPD. The Respiratory Therapist works closely with your Nurse Practitioner to recommend an individualized treatment plan.
Youth Wellness Hub
In 2025, LNPLC established a new partnering with the Youth Wellness Hub to expand access to primary health care services for youth between the ages of 12 and 25. Through this initiative, LNPLC provides on-site primary care services designed to support the physical, mental, and emotional well-being of youth in a welcoming and accessible environment. Services include primary care assessments, preventative health services, chronic disease management. mental health support, sexual health services, health education. and referrals to community and specialized supports as needed. This partnership reflects LNPLC’s commitment to improving access to integrated, youth-centred care with in the community.
Lakehead Superior Connect Program
LNPLC provides care in collaboration with Superior Emergency Medical Services to support vulnerable patients living with chronic health conditions, including Congestive Heart Failure (CHF), Chronic Obstructive Pulmonary Disease (COPD), diabetes, and frailty. Through this innovative partnership, community paramedics conduct home visits and work closely with Nurse Practitioners to coordinate care and deliver comprehensive primary health care services in patients’ homes and within the clinic setting. Patients benefit from access to an interdisciplinary team that includes a dietitian, social worker, smoking cessation counselor, pharmacist, and kinesiologist, ensuring a holistic and patient-centred approach to care. The availability of both home visits and in-clinic appointments provides flexibility for patients while helping to improve health outcomes, reduce barriers to care, and support individuals in managing complex health conditions within their communities.
SAC
In 2025, Ontario Health Minister Sylvia Jones and Dr. Jane Philpott announced a provincial commitment to ensure that every Ontarian is connected to a family physician or Nurse Practitioner within the next four years. Achieving this goal requires strong collaboration among all primary care professionals and organizations across the health care system. The Lakehead Nurse Practitioner-Led Clinic (LNPLC) is proud to support this initiative and has partnered with other community providers to host Thunder Bay’s Supported Attachment Centre, a specialized program staffed by a dedicated team of system navigators who work closely with Health Care Connect to help unattached patients find access to primary care providers within the community.
To date, the Supported Attachment Centre and Health Care Connect have successfully cleared al patients registered with Health Care Connect prior to January 2025 providing attachment for those in need. This achievement reflects the strong partnership among clinics and primary care providers across Thunder Bay and demonstrates how collaborative community-based solutions can help address local health care challenges and improve access to care for residents.
