Programs & Services
Nurse Practitioners provide comprehensive, accessible, and coordinated family health care services through Nurse Practitioner-Led Clinics to populations who have difficulty accessing a primary care provider (i.e. unattached patients). Nurse Practitioner-Led Clinics deliver:
- Comprehensive primary care to patients in the community ensuring continuity of care;
- Holistic care that addresses all needs of each patient; Enhanced health promotion;
- Chronic Disease and illness management and prevention;
- Care coordination and navigation of the health care system;
- Walk-In Clinic for LNPLC patients – Mon-Sat
- Integrated care within the community through established partnerships.

Prescription To Get Active
Your provider can write you a prescription to keep your physical activity level at the standard for basic fitness and health. Register your Prescription here: Prescription To Get Active and you can access a huge amount of fitness resources, as well as a one-month trial membership at the Canada Games Complex.
Skin Screening
If you need something checked on your skin, please make an appointment with your Nurse Practitioner. We offer cryotherapy to treat warts, skin tags and certain types of moles.
Mindful Eating
This 6-week interactive cognitive behavioral therapy-based group program is designed to help you better understand and manage your food cravings while healing your relationship with food.

Concerned about possible memory issues? Speak with your Nurse Practitioner about Memory Screening. We are here to help.
Caregiver Support
Are you the caregiver for a patient of our clinic, or a family member? It will be hosted by a number of our clinic’s providers including Social Worker, Dietitian, NP, and local organizations.

Adult Mental Health
Join a workshop for older adults (65+) where you’ll discuss what is mental health, mental illness and addiction, what can affect the mental health of older adults and how can you stay well.
Advance Care Planning
Talking about end of life care is not easy, but if you want your wishes for future health care and treatment to be known, you need to talk to your loved ones, Substitute Decision Maker and anyone else involved in your care.
