Physiotherapist/Kinesiologist:

Lakehead NPLC is seeking a part-time physiotherapist or kinesiologist to work in collaboration with primary care providers to address patient concerns in mobility and injury recovery with an emphasis on frailty and chronic disease management. Registered Social Worker:

Are you a dedicated and compassionate Social Worker with a
passion for health promotion and disease prevention? Join our team at the
Lakehead Nurse Practitioner-Led Clinic (LNPLC) as a Social Worker and make a
meaningful impact in the lives of individuals, couples, and families in our
community. In this role, you will work collaboratively with our team of Nurse
Practitioners, Registered Nurses, Registered Practical
Nurses, Social Workers, Registered Dietitian, and Pharmacist
to provide
essential counselling, system navigation, and support services to a rostered
population of patients in the city of Thunder Bay.  This is a part-time contract position that will run until March 31, 2025 with an option to renew. Hours are flexible and may include some evenings. 

Key Responsibilities:

  • Represent LNPLC with professionalism, aligning with the organization’s mission and values.
  • Provide short-term, goal-directed counselling to clients.
  • Conduct screenings and assessments related to health promotion, disease prevention, and chronic disease management.
  • Facilitate community referrals and support client access to necessary services.
  • Collaborate with the healthcare team to plan, facilitate, and evaluate group programs.
  • Advocate for social change and build effective partnerships with community agencies.
  • Ensure compliance with legislation, standards of practice, and confidentiality policies.
  • Maintain accurate records, participate in case consultations, and engage in professional development.

Education, Qualifications, and Skills:

  • HBSW or higher with registration in good standing with the OCSWSSW
  • 5 years of experience in counselling/clinical role.
  • Excellent interpersonal, communication, and clinical skills.
  • Proven ability to teach, counsel, and develop educational materials.
  • Strong understanding and knowledge of community resources

Summer 2024 LNPLC is trialing new AI scribe software that will reduce the administrative burden on clinicians, allowing them to spend more time with clients, improve communication, reduce wait times, and reduce stress. This all adds up to a better experience for our patients. This trial is being supported by our regional Ontario Health Team Noojmawing Sookatagaing so that we can share our experience with other healthcare organizations and assist with faster access to care across the District of Thunder Bay. 

Starting in September, 2024 LNPLC will choose one vendor to work with and implement this dictation assistance in appointments regularly. For more information about this trial and our digital improvements please contact the clinic.

 

As of April 1, 2024, telephone reminders will no longer be provided except for program offerings. Lakehead NPLC has implemented a secure messaging portal called Medeo which will offer email reminders 24 hours prior to appointments, allow faster communications to complete forms, inform negative test results, and other information initiatives. Through February to April 2024, LNPLC’s social work student will ask you to complete a short questionnaire for your health record about informing the Clinic of your Power(s) of Attorney (POAs) and/or your Substitute Decision Maker(s) (SDMs).

The Lakehead Nurse Practitioner-Led Clinic is pleased to welcome Liuba, a social work student from Lakehead University. She will be asking you about updating your health record to include an Ontario-wide initiative called Advance Care Planning. Through February to April 2024, Liuba will be at reception on Mondays, Tuesdays, and Thursdays to ask you to fill out a short questionnaire. It will be about your substitute decision maker(s) and/or power(s) of attorney for personal care in the event that you are sick or injured and could not tell your nurse practitioner what kind of care you wanted.This is a scary thing to think about, but thinking about it today will help bring you peace of mind for the future!

If you would prefer to fill out the questionnaire at home and return it to the clinic on your next visit, you can print it off and hand it in to any clinic staff, at reception, or you can complete the fillable PDF form and email it to info@lnplc.ca. If you have any questions, Liuba is happy to answer them for you when she is at reception.
